Gameplay Questionnaire - as part of some research

It would be greatly appreciated if you fill this questionnaire out.
It's about games and gameplay. It's for some research that is part of my Computer Science coursework. The idea of the survey is to get opinions from a potential end-user of the software I produce.
The coursework allows us to program basically anything we want - I've chosen a non-linear game for mine. Your opinions on gameplay will greatly help with my work, so it'd be really helpful if you took 5 minutes or so to fill it out! Thanks again.
Official Level Reviewer (My Gold Yoshi Egg rank replaced the level reviewer icon, but i am still a level reviewer).

You should take an example from Touhou Luna Nights. It has great game mechanics, charming graphics, and a variety of themes.
